Registration Information:

A medical form must be filled out prior to the acceptance of any fees or placement of child on the camp roster.  Please have doctors / dentists names and phone numbers handy. Although it is not mandatory, we would appreciate it if you had the medical form notarized.  You may pick up registration and medical forms in advance, so that you can have them filled out prior to registration.  Forms will be mailed out to those that participated in the 2009 summer camp. (A doctor’s signature is not required.)

There is a limit to the number of registrants due to space limitations and safety factors.  Your child must be 5 years of age and no older than 12 years old by the first day of camp.  A birth certificate or copy of it is necessary if he or she is entering kindergarten.

On the day of registration, full payment for the first session your child attends is due.  Following sessions must be reserved with a NONREFUNDABLE $25.00 deposit per session.  Bus service to and from camp will be available for each session.  Full payment for bus service for all sessions will be due when you register, and are NONREFUNDABLE.

Adventure Camp has a limited number of spaces due to the extensive amount of field trips, therefore, registration is taken on a first come, first serve basis.
