Patient Access Concierge offers more personalized experience for University Hospitals Geauga Medical Center patients

New role optimizes single point of contact to schedule tests, appointments, inpatient procedures

GEAUGA COUNTY – University Hospitals (UH) Geauga Medical Center has launched its new Patient Access Concierge program, now available to patients and family members each weekday. The Concierge is located in the Outpatient Services registration area on the first floor of the hospital.

The program offers an even more convenient and personalized experience for patients who come to UH Geauga Medical Center. The Concierge provides face-to-face communication and assistance in scheduling follow-up physician appointments, inpatient procedures, outpatient testing and other arrangements for clinical care including primary care referrals.

Accessibility and a commitment to compassionate caring are among the Concierge’s priorities. “I am the patients’ link between our hospital and the clinician and I’m here to help them have their immediate clinical needs fulfilled with personalized service,” says Bonnie Zachowski, CHAA, Patient Access Concierge, UH Geauga Medical Center. “For example, when a cancer patient has an immediate medical need to be precertified for a CT or PET scan, I am here to help make that happen quickly and efficiently.”

Zachowski, who has been with UH Geauga Medical Center for two years and University Hospitals for 14 years, became the community hospital’s Patient Access Concierge in March. Her role is an in-person extension of UH Geauga Medical Center’s “single point of contact” philosophy, which was established with the launch of the Geauga Patient Call Center.

On behalf of each patient, the Call Center team works hand-in-hand with many different departments at UH Geauga Medical Center; the hospital’s Outpatient Rehabilitation Services and Aquatic Center; and UH Concord Health Center. “With one phone call a patient can schedule appointments and complete any necessary registration for a procedure or test,” says Denise L. Toth, CHAA, Patient Access Manager, UH Geauga Medical Center. “As our Concierge, Bonnie takes that single point of contact a step further with her accessibility to patients who are physically at the hospital.”

The Patient Access Concierge office hours are Monday – Friday, 8 a.m. – 4:30 p.m. UH Geauga Medical Center patients can also reach Bonnie Zachowski by calling 440-285-3036 during those hours.

The Geauga Patient Call Center can be reached Monday – Friday, 8 a.m. – 5:30 p.m. at 440-285-3030 or 1-855-GEAUGA (1-855-432-8421).
